2. Collect Participant Saliva Sample

Hand saliva collection tube package to participant so that they can provide saliva sample as shown in instructions below

Make sure participant knows to reference their unique code for the required questionnaire and to access their microbiome results

Store capped saliva sample in designated clinic location for pickup by study personnel

Make sure to place saliva samples and completed paper copies of the consent forms in the designated clinic area for study personnel to collect
3. Clinic Survey

The Clinic Survey is how you will report any previous or currently diagnosed periodontal disease staging or known presence of an abscess infection for the participant
